Job Summary
We are seeking a dedicated ACT Generalist to join our dynamic mental health team. In this vital role, you will deliver comprehensive Assertive Community Treatment (ACT) services to individuals experiencing severe mental health challenges. Your energetic and compassionate approach will empower clients to achieve stability, independence, and improved quality of life. As an ACT Generalist, you will coordinate multidisciplinary care, provide therapeutic interventions, and foster a supportive environment that promotes recovery and resilience.
Responsibilities
- Deliver personalized, evidence-based mental health services directly in community settings to meet clients where they are.
- Conduct thorough assessments in person, weekly
- Collaborate with a multidisciplinary team to coordinate care, ensuring seamless support.
- Engage clients actively in their recovery process through motivational interviewing, psychoeducation, and skill-building activities.
- Monitor client progress regularly, adjusting interventions as needed to maximize positive outcomes.
- Document all interactions meticulously, maintaining accurate records of treatment plans, progress notes, and service delivery.
Experience
- Prior experience working in community mental health or similar settings with a focus on severe mental illness is preferred.
- Familiarity with psychopharmacology principles and medication management strategies.
- Strong interpersonal skills with the ability to build trusting relationships.
- Ability to work independently and as part of a collaborative team in fast-paced environments.
